IMPORTANT WAIVER INFORMATION
One waiver per person is required.
Guests under 18 years of age must have their waiver signed by a parent or legal guardian at the bottom of the form.
No changes, edits, or cross-outs are permitted on the waiver form. Any altered waivers will be considered invalid.
Waivers must be completed in full to be accepted. Incomplete forms will delay or prevent your participation in the tour.
BEFORE YOU BEGIN, MAKE SURE YOU HAVE THE FOLLOWING INFORMATION:
Trip Location: Canada or USA (Where are you taking your tour?)
Boat Type: Wet Jet, DRY Freedom Jet, WET Freedom Jet
Order Number: 6-digit number from your confirmation email or booking receipt.
- Last Name or Group Name on Reservation: The name the booking was made under.
- Trip Date: MM/DD/YYYY format.
- Trip Time: Your scheduled departure time — be accurate so we can match you properly!
